Preparing Your CS Form 212 (Personal Data Sheet) as a PDF (Philippines)

Applying for a government position in the Philippines usually involves the Civil Service Commission's Personal Data Sheet, known as CS Form 212, along with other supporting documents. The form is widely distributed as a spreadsheet, so many applicants are unsure how to turn it into a clean PDF. This guide walks through that process without going beyond what the hiring agency itself asks for.

Use the current version of the form

The CSC has published a revised version of the Personal Data Sheet that replaces the earlier 2017 form. At the time of writing, the current one is labelled Revised 2025. Agencies expect applicants to use the latest version, and an outdated form can cause delays. Always download it from the CSC's official site or the agency's own vacancy announcement, and follow the announcement if it names a specific version.

Fill it in completely and accurately

Complete every section that applies to you and write not applicable where it does not, rather than leaving a blank. Accuracy matters more than speed: agencies can treat false or inconsistent information seriously. Check names, dates and numbers against your supporting documents before you convert anything, so the PDF you produce is the version you intend to submit.

Convert the spreadsheet to PDF

If you filled in the form as an Excel file, create the PDF from Excel itself: choose File, then Save As or Export, and pick PDF. That keeps the boxes, lines and page layout of the official form. The Excel to PDF tool on this site is not suited to this job, because it converts a sheet into a plain data table and does not keep a form's layout. Open the PDF and scroll through every page to make sure nothing was cut off and the page breaks look right. Sign, combine and shrink the file

Where the agency accepts a scanned or electronic signature, the sign PDF tool can place a signature image on the form. The most frequent errors are using an outdated version of the form, leaving fields empty instead of writing not applicable, and submitting a PDF whose pages were cut off during conversion. Another is sending a file that does not match what the announcement asked for, such as the wrong file name or format.

Read the vacancy announcement twice before you submit, and tick off each required document as you attach it. If you are unsure about anything, ask the agency before the deadline instead of assuming.

Exporting every sheet of the workbook

The Excel version of the form is a workbook with one sheet per page. When you save as PDF, Excel exports only the sheet you are looking at unless you tell it otherwise. In the save or export window, open Options and choose the entire workbook, or export each sheet and join them in page order with Merge PDF. Count the pages in the finished PDF against the number of sheets.

If a column of the form spills onto an extra page, do not shrink the text by hand. Check the page setup in Excel is still set as the CSC published it, and export again.

If you printed and signed on paper

Many applicants print the form, sign it, attach a photo and add a thumbmark by hand, then need a PDF to email. Photograph each page flat and in good light, and turn the photos into a PDF with Scan to PDF, in page order. The tool places photos as taken, so trim any background with Crop PDF and check the signature, photo and thumbmark are clear. A scan like this compresses well if the agency's email system limits attachment size.

Putting the attachments in order
